About

Healthy Together is Tasmanian Government initiative that aims to improve the health and wellbeing of the community but led by the community.

With Hobart City Mission as the service provider, and a Project Officer working within the communities to help improve health and wellbeing by making decisions together, involving communities, and supporting leadership that encourages change.

The plan

Community focused programs to support connections to people, place and culture and promote healthy and active lifestyles.

We are seeking information from all communities within the Southern Midlands. An advisory group has been formed with representatives from each community.

Together we will plan a way forward to ensure all community members are given opportunities to ensure their voices are heard and their health and wellbeing is being supported.

This website was created in response to the community’s wishes and you will find more information about the projects in which community and Healthy Together are collaborating by clicking on the ‘News’ button.

The need for a community directory was highlighted in early conversations with communities. This website has been created in response to this feedback.

Surveys

To ensure Healthy Together have a broad understanding of each community’s needs, surveys have been going out into communities. Alongside many conversations with community members and contacts with community representatives. The ones that have been received so far are starting to give Healthy Together a deeper understanding of community needs and suggestions. This feedback is helping to build a map of each community’s assets and what they feel is missing. Healthy Together will be sharing the information gathered on the website, sent back to communities, on social and local media plus all local service providers. Thus far we have place surveys in the following townships, Bagdad, Kempton, Oatlands, Tunnack and Colebrook.
